Which interpretation best fits this presentation?

Numbers stratify risk; trajectory decides what to do next

Severity tools organize prognosis but should be interpreted with bleeding, infection, kidney function, and diagnostic certainty.

Current ACG guidance uses MELD above 20 to identify severe alcohol-associated hepatitis for corticosteroid consideration when no major contraindication remains.

A Maddrey discriminant function of at least 32 is a historical severity threshold, but it should not displace MELD, infection assessment, kidney function, or diagnostic certainty.

The Lille score on day 4 or 7 estimates corticosteroid response; a value above 0.45 supports stopping because benefit is unlikely.

Mild steatosis without dysfunction

Fat, active hepatitis, and scar are different clinical states

Alcohol exposure can produce overlapping phenotypes, but each state has a different short-term risk.

Simple steatosis is often asymptomatic and reversible with abstinence; it does not require jaundice or synthetic dysfunction.

Alcohol-associated hepatitis is an acute inflammatory syndrome marked by recent or worsening jaundice, often with tender hepatomegaly, fever, leukocytosis, and impaired synthetic function.

Cirrhosis reflects advanced fibrosis and portal hypertension; a patient may have normal or only mildly abnormal AST and ALT despite major risk.

Compare the defining clinical payoff of each phenotype.

Macrovesicular fat may cause hepatomegaly and mild enzyme abnormalities; abstinence can reverse fat before fibrosis is established.

Normal aminotransferases do not rule out cirrhosis.

Build the diagnosis before reaching for prednisolone

The safe sequence identifies the syndrome, grades severity, and actively searches for treatment hazards.

Confirm alcohol exposure timing and quantify intake without using stigma-laden labels.

Exclude biliary obstruction, viral hepatitis, drug injury, ischemia, and other plausible causes; use biopsy when the diagnosis remains uncertain and would change treatment.

Assess bilirubin, INR or prothrombin time, creatinine, sodium, mental status, infection, gastrointestinal bleeding, and nutrition.

AST and histology support; they do not certify

Classic clues become dangerous when treated as stand-alone diagnostic tests.

AST commonly exceeds ALT in alcohol-associated hepatitis, often with both below 400 U/L, but the ratio is not specific for alcohol and can change with advanced disease.

Mallory-Denk bodies are damaged keratin aggregates within ballooned hepatocytes. They support steatohepatitis but can occur in metabolic steatohepatitis, cholestatic disease, and other injury.

Aminotransferase magnitude measures hepatocyte leakage, not hepatic reserve; bilirubin, INR, creatinine, and clinical decompensation better express severity.

Map injury from hepatocyte to whole-patient reserve

The pathology and the bedside findings occupy different levels of the same disease.

Steatosis and ballooning begin in hepatocytes, while neutrophilic inflammation and pericellular fibrosis often cluster in zone 3.

Progressive bridging fibrosis remodels the liver into cirrhosis, raising portal resistance and producing splenomegaly, varices, and ascites.

Extrahepatic danger includes infection, acute kidney injury, sarcopenia, alcohol withdrawal, and malnutrition.

Locate each finding on the disease map.

Steatosis, ballooning degeneration, and Mallory-Denk bodies are cellular injury findings.

Treatment is layered, and steroids belong to a narrow layer

Every patient needs alcohol-use treatment and nutrition; only selected patients with severe hepatitis are corticosteroid candidates.

Use nonjudgmental addiction treatment, pharmacotherapy when appropriate for liver status, and longitudinal relapse prevention. Sustained abstinence is the most important disease-modifying intervention.

Correct protein-calorie and micronutrient deficits; do not routinely restrict protein because sarcopenia worsens outcomes.

Prednisolone 40 mg daily for 4 weeks may be used for severe alcohol-associated hepatitis, generally MELD above 20, after major contraindications are addressed; stop when the day 4 or 7 Lille score is above 0.45.

Reveal what each intervention can and cannot accomplish.
