Fractionate bilirubin promptly in a persistently jaundiced infant and assess liver enzymes, GGT, synthetic function, glucose, infection, and treatable metabolic causes in parallel.

Ultrasound evaluates the gallbladder, triangular cord region, ductal anatomy, choledochal cyst, and associated anomalies. Liver biopsy can support obstruction and evaluate mimics, while intraoperative cholangiography establishes whether an extrahepatic biliary tree is patent.

Recognize cholestasis

Use direct bilirubin, dark urine, and pale stool rather than visual jaundice alone.

Follow bile from hepatocyte to diaper

The color of urine and stool localizes what happened to conjugated bilirubin.

Hepatocytes conjugate bilirubin and secrete it into canaliculi, which drain through intrahepatic ducts, the hepatic ducts, and the extrahepatic biliary tree before bile reaches the intestine.

When the extrahepatic ducts are obliterated, conjugated bilirubin refluxes into blood and is filtered into urine, while little bilirubin pigment reaches stool. Progressive retained bile injures and scars the liver.

Place each clue along the route.

Conjugation occurs, so the circulating excess is water-soluble direct bilirubin.

Separate indirect jaundice from neonatal cholestasis

The bilirubin fraction and stool color divide a benign transition from an obstructed bile route.

Physiologic jaundice begins after the first day of life, is predominantly unconjugated, and occurs in an otherwise well infant with pigmented stools and no bilirubinuria.

Biliary atresia causes persistent conjugated hyperbilirubinemia, pale or acholic stools, dark urine, and progressive hepatomegaly. Neonatal hepatitis and other intrahepatic disorders can also cause conjugated jaundice, so the phenotype triggers an urgent evaluation rather than proving one cause.

Compare the three neonatal patterns.

Physiologic jaundice

Indirect bilirubin predominates; stools remain pigmented and urine does not contain bilirubin.

Biliary atresia

Direct bilirubin, acholic stools, dark urine, firm hepatomegaly, and often high GGT point to obstructed bile flow.

Neonatal hepatitis

Direct bilirubin with hepatocellular inflammation may have pigmented or variably pale stools and broader infectious, metabolic, or genetic clues.

Do not call any conjugated neonatal jaundice physiologic.

Know what each test cannot prove

Biliary atresia is missed when a supportive test is mistaken for a gatekeeper.

GGT is often elevated in biliary atresia but is neither diagnostic nor uniformly abnormal in every cholestatic disorder.

Failure of tracer to reach the bowel on HIDA supports impaired bile flow but has limited specificity because severe intrahepatic cholestasis can also cause nonexcretion. Demonstrated excretion argues against complete obstruction, but a HIDA study must not delay definitive evaluation.

HIDA can support the map; it does not own the diagnosis.

Let age and physiology set urgency

A stable-looking infant can still be losing the best surgical window.

The AAP identification strategy focuses on the 2- to 4-week visit, asking about persistent jaundice, pale stools, and prior direct or conjugated bilirubin results.

Earlier Kasai is associated with better native liver outcomes. Urgency rises further with coagulopathy, hypoglycemia, poor growth, infection, or signs of portal hypertension.

Restore flow early and plan beyond the Kasai

Portoenterostomy is a time-sensitive first operation, not a guaranteed cure.

Kasai portoenterostomy removes the fibrotic extrahepatic remnant and connects a Roux limb to the porta hepatis. Outcomes are better when surgery occurs early, with the strongest emphasis on referral before 30 to 45 days and performance before about 60 days whenever possible.

After Kasai, care includes nutrition, fat-soluble vitamin support, monitoring for cholangitis and portal hypertension, and serial assessment of bile drainage and growth. Transplant is required for failed drainage, progressive cirrhosis, recurrent uncontrolled cholangitis, growth failure, portal-hypertensive complications, or liver failure.
