Cirrhosis, abdominal malignancy, pancreatitis, infection, surgery, and inherited or acquired thrombophilia can predispose to portal vein thrombosis.

Outflow block versus inflow block

Budd-Chiari syndrome is obstruction of hepatic venous outflow, usually from hepatic vein thrombosis and sometimes from the adjacent inferior vena cava.

Portal vein thrombosis blocks portal inflow; acute extension into mesenteric veins can threaten bowel, while chronic disease can form cavernous collaterals.

Budd-Chiari syndrome

Outflow obstruction causes sinusoidal congestion, tender hepatomegaly, ascites, and liver dysfunction.

Name the blocked vessel before naming the syndrome.

Read the scan as a flow diagram

Localize the obstruction from gut to liver to vena cava.

Portal blood enters at the porta hepatis, crosses sinusoids, drains through hepatic veins, and reaches the inferior vena cava.

Cavernous transformation describes serpiginous periportal collateral channels that develop around chronic portal vein obstruction.

Extension here raises concern for intestinal congestion and ischemia.

The same thrombus location has different consequences as extension and organ dysfunction increase.

Acute portal-mesenteric extension carries greater immediate intestinal risk than stable chronic cavernous transformation.

Progressive liver failure despite anticoagulation and decompression moves Budd-Chiari syndrome toward transplant evaluation.

Treatment escalates with physiology

Anticoagulation addresses thrombosis, but persistent congestion or organ failure requires more.

Unless contraindicated, anticoagulation is central for Budd-Chiari syndrome and recent noncirrhotic portal vein thrombosis, with duration shaped by reversibility and ongoing thrombophilia.

In cirrhosis, recent limited thrombosis can be observed with imaging every 3 months, whereas recent more than 50 percent occlusion or main portal or mesenteric involvement favors anticoagulation; chronic complete cavernous transformation is not routinely anticoagulated.

Budd-Chiari management can progress from anticoagulation and treatment of complications to angioplasty or stenting, TIPS, and liver transplantation.
