The route distinguishes production, transport, conjugation, excretion, and downstream clues.

A 17-year-old develops mild scleral icterus during influenza and poor oral intake. Total bilirubin is 3.1 mg/dL, almost entirely unconjugated; CBC, reticulocytes, AST, ALT, and alkaline phosphatase are normal.

Which diagnosis is most likely?

Grade the UGT1A1 spectrum

The same conjugation step produces three very different clinical burdens.

Gilbert syndrome retains substantial UGT1A1 activity and causes mild, intermittent, isolated unconjugated hyperbilirubinemia, often first recognized in adolescence.

Crigler-Najjar type I has absent or nearly absent activity, presents in infancy with severe persistent unconjugated hyperbilirubinemia, does not respond to phenobarbital, and carries a high kernicterus risk. Type II retains partial activity, is less severe, and usually lowers bilirubin with phenobarbital.

Compare phenotype, response, and risk.

Mild intermittent indirect bilirubin, normal liver tests, no progressive liver disease, and no routine treatment.

Phenobarbital response separates residual enzyme from no useful enzyme.

Trace bilirubin to the blocked chemistry

A normal liver can still fail at one conjugation reaction.

Heme breakdown produces lipid-soluble unconjugated bilirubin, which travels bound to albumin, enters hepatocytes, and is glucuronidated by UGT1A1.

Conjugation makes bilirubin water soluble for biliary excretion. Reduced UGT1A1 leaves more unconjugated bilirubin in plasma; unbound bilirubin can cross the immature or stressed blood-brain barrier and injure deep brain nuclei.

Read the bilirubin as a severity range, not a lone label

The magnitude and persistence help place the patient on the UGT1A1 spectrum.

Gilbert syndrome usually remains below about 4 mg/dL and fluctuates with triggers. Crigler-Najjar type II often falls in an intermediate range, commonly below 20 mg/dL, while untreated type I is often 20 to 50 mg/dL.

Ranges overlap and treatment decisions depend on age, albumin binding, illness, neurologic signs, and the trajectory, not a single cutoff.

Use triggers and phenobarbital as discriminators

A trigger unmasks Gilbert; an inducer reveals residual UGT1A1.

Fasting, dehydration, illness, strenuous exertion, and menstruation can transiently increase bilirubin in Gilbert syndrome. Provocative fasting tests are unnecessary and are no longer recommended.

Phenobarbital induces residual UGT1A1 activity and substantially lowers bilirubin in Crigler-Najjar type II. Type I lacks a clinically meaningful response.

Persistent indirect hyperbilirubinemia that falls substantially with phenobarbital

Type II moves with phenobarbital; type I does not.

Protect the brain from unbound bilirubin

The danger is not the yellow color; it is bilirubin leaving albumin and entering neural tissue.

Unconjugated bilirubin is lipid soluble and normally circulates tightly bound to albumin. Very high concentrations, acidosis, sepsis, prematurity, or albumin-displacing drugs can increase the unbound fraction.

Bilirubin neurotoxicity classically affects the basal ganglia and selected brainstem nuclei, producing acute lethargy, poor feeding, abnormal tone, arching, or seizures and potentially chronic kernicterus with movement, hearing, and gaze abnormalities.

Match treatment intensity to residual activity

Management ranges from reassurance to a bridge toward liver replacement.

Gilbert syndrome needs reassurance, trigger counseling, and attention to drugs affected by UGT1A1, not chronic bilirubin-lowering therapy.

Crigler-Najjar type II is usually controlled with phenobarbital plus monitoring, while illness or pregnancy can narrow the safety margin. Type I requires prolonged intensive blue-light phototherapy, often for many hours daily, with urgent rescue measures for dangerous rises and liver transplantation before irreversible kernicterus; phototherapy becomes less efficient as body surface area relative to mass falls with age.
