What is the most important interpretation?

Measure before discharge, then plan the handoff

Visual inspection alone cannot safely estimate treatment proximity.

The AAP 2022 guideline recommends at least one transcutaneous or total serum bilirubin measurement for every infant before discharge, generally at 24 to 48 hours or earlier if discharge occurs sooner.

Interpret the result by postnatal age in hours, gestational age, and hyperbilirubinemia neurotoxicity risk factors, then schedule follow-up from the distance below the phototherapy threshold.

Assess jaundice and risk repeatedly

Check feeding, weight trajectory, gestational age, bruising, family history, ancestry-related G6PD risk, and illness.

Follow bilirubin to the brain or bile duct

Unconjugated and conjugated bilirubin create different hazards and clues.

Unconjugated bilirubin circulates albumin bound; when exposure is excessive or binding is impaired, bilirubin can cross the blood-brain barrier and injure basal ganglia and brainstem nuclei.

Conjugated bilirubin is water soluble, can darken urine, and signals cholestasis rather than physiologic transition; pale stools raise concern for impaired bile flow.

Separate transition, intake, milk effect, and disease

Timing and infant wellness prevent feeding labels from hiding pathology.

Physiologic unconjugated bilirubin rises after the first day because bilirubin production is high and hepatic uptake and conjugation are still maturing.

Suboptimal-intake hyperbilirubinemia occurs early with inadequate milk transfer and excess weight loss; breast-milk jaundice is later and prolonged in an otherwise thriving infant.

Early and underfed is not the same as later and thriving.

Red flags outrank reassurance

A single red flag moves the infant from routine transition to active evaluation.

First-day jaundice, a rapid bilirubin rise, anemia, lethargy, poor feeding, fever, dark urine, pale stools, or a conjugated component is abnormal.

Hemolysis may reflect ABO or Rh incompatibility, hereditary red-cell disease, or G6PD deficiency; oxidative stress can trigger abrupt severe hyperbilirubinemia in G6PD deficiency.

Three axes determine the treatment line

The raw bilirubin concentration is incomplete without age, maturity, and neurotoxicity risk.

Lower gestational age and neurotoxicity risk factors such as hemolytic disease, G6PD deficiency, sepsis, significant clinical instability, or low albumin lower treatment thresholds.

A bilirubin result only slightly below the phototherapy threshold predicts a greater chance of crossing it and warrants earlier follow-up than a result far below the line.

AAP thresholds incorporate developmental vulnerability and clinical risk.

Phototherapy is started when the hour-specific threshold is reached for the infant's gestational age and neurotoxicity risk profile; escalation-of-care and exchange-transfusion thresholds are higher but demand urgent action.

Optimize feeding and lactation support, treat hemolysis or infection, avoid oxidative triggers in G6PD deficiency, and never use phototherapy as treatment for conjugated hyperbilirubinemia.

Jaundice in the first 24 hours is pathologic until evaluated for hemolysis and other causes. Very early jaundice and possible ABO incompatibility require prompt bilirubin measurement and hemolysis evaluation.

Why can bilirubin rise?
