A 19-year-old woman with type 1 diabetes mellitus is brought to the emergency department for one day of nausea, repeated vomiting, abdominal pain, and rapid breathing. She ran out of insulin two days ago. Temperature 37.2 C (99.0 F), heart rate 124/min, blood pressure 92/58 mm Hg, respiratory rate 28/min, oxygen saturation 99% on room air. She is drowsy but arousable with dry mucous membranes. After 1 L of 0.9% saline she has produced 70 mL of urine. Labs: glucose 562 mg/dL, sodium 129 mEq/L, potassium 3.0 mEq/L, chloride 92 mEq/L, bicarbonate 8 mEq/L, anion gap 29, beta-hydroxybutyrate 6.8 mmol/L, arterial pH 7.11.
Which of the following is the most appropriate next step in management?

First, she is breathing fast and her blood is acidic. What is diabetic ketoacidosis, in plain words?
Her body has almost no insulin, so it cannot push glucose into cells. Starving cells switch to burning fat, which spills acidic ketones into the blood. The lungs blow off CO2 fast to compensate. That rapid breathing is the body fighting the acid.
Now add her potassium of 3.0 and a day of vomiting. Why is that single lab the one that decides what happens next?
Insulin drives potassium into cells. Start it now and her potassium can crash into a lethal arrhythmia. Vomiting and sugar-heavy urine have already drained her potassium, so the tank is empty before you add the drug that empties it further.
The obvious move is to fix the acid and the glucose. Why is neither bicarbonate nor insulin the first step for her?
Bicarbonate does not help in routine diabetic ketoacidosis. The acid clears once fluids and insulin restore metabolism. Insulin is exactly the drug that would drop her potassium, so it waits. The number blocking both is the same: potassium 3.0, under the 3.5 threshold where insulin becomes dangerous.
She is volume-depleted and her potassium is 3.0. What is the sequence, and what single number locks it in?
Continue isotonic fluids, give intravenous potassium chloride first, then start insulin once potassium is above 3.5 mEq/L. The 3.5 threshold is the gate. Below it, insulin can kill. Above it, insulin is safe. She is below it, so potassium comes first.
One tempting answer is to start the insulin drip now because her glucose is 562 and her pH is 7.11. Why is that the dangerous move for her?
Her potassium is already 3.0. Insulin shifts potassium into cells, and her serum potassium could fall fast enough to trigger ventricular arrhythmia and cardiac arrest. The emergency is the potassium, not the glucose. Fix it first, and the insulin drip becomes safe.
