Know who is average risk

Screening guidance applies to average-risk asymptomatic adults; high-risk conditions require a separate plan.

The USPSTF recommends colorectal cancer screening beginning at age 45 for average-risk adults and continuing routinely through age 75.

For ages 76 through 85, screening is selective and depends on prior screening, overall health, life expectancy, and preferences.

Positive non-colonoscopy screening tests require follow-up colonoscopy to realize the benefit of screening.

Move from suspicion to stage

Symptoms change the task from screening to diagnosis, and treatment planning begins only after tissue and extent are established.

Colonoscopy visualizes the lesion, obtains biopsy tissue, and evaluates the remaining colon for synchronous neoplasia.

After adenocarcinoma is confirmed, stage with appropriate cross-sectional imaging and determine local invasion, regional nodes, distant spread, and whether metastatic disease is resectable.

Obtain a baseline CEA after diagnosis when useful for later comparison, but never use a normal CEA to exclude cancer.

Read the side from the symptom

Tumor location predicts the earliest clinical signal because luminal caliber and stool consistency differ across the colon.

Right-sided cancers grow in a capacious lumen with liquid contents and commonly cause occult blood loss, iron-deficiency anemia, fatigue, and weight loss before obstruction.

Left-sided cancers narrow a smaller lumen containing formed stool and more often cause change in bowel habits, decreasing stool caliber, cramping, hematochezia, or obstruction.

Either side can bleed, obstruct, metastasize, or present nonspecifically; laterality is a probability tool, not a rule-out test.

Right colon

Occult bleeding, iron-deficiency anemia, fatigue, and a later palpable mass; obstruction tends to occur later.

Right bleeds silently; left blocks loudly.

Map the major risk neighborhoods

Risk rises through acquired exposures, chronic mucosal inflammation, prior neoplasia, and inherited susceptibility.

Colorectal cancer is one of the most common cancers worldwide; incidence rises with age, although younger-onset disease is increasing.

Advancing age is a major population risk, although incidence in younger adults has increased and symptoms at any age still require evaluation.

Important personal risks include adenomatous polyps or prior colorectal cancer, long-standing colonic inflammatory bowel disease, obesity, physical inactivity, smoking, and higher alcohol or processed-meat exposure.

Family history and syndromes such as Lynch syndrome and familial adenomatous polyposis can justify earlier and more intensive surveillance than average-risk schedules.

Place each risk signal in its clinical neighborhood.

Older age, obesity, inactivity, tobacco, alcohol, and dietary patterns associated with colorectal carcinogenesis.

Scale treatment with disease extent

The curative core is complete resection when disease is localized and resectable; systemic therapy gains importance with nodal and distant spread.

Stage I colon cancer is generally treated with oncologic surgical resection without routine adjuvant chemotherapy.

Stage II colon cancer is resected; adjuvant therapy is individualized for high-risk features, and mismatch repair status informs the discussion.

Stage III colon cancer generally receives resection plus adjuvant oxaliplatin-based chemotherapy when the patient can tolerate it.

Stage IV care depends on resectability and molecular findings: selected limited metastases may be resected or ablated, while unresectable disease receives biomarker-directed systemic therapy, including immunotherapy for MSI-H or dMMR tumors.

For selected metastatic regimens, induction is the initial intensive phase used to gain control or enable resection; maintenance is a planned lower-intensity continuation after response or stable disease. Adjuvant therapy after curative resection is time-limited and is not maintenance.

Reveal what each test can and cannot do

The common error is assigning a screening, diagnostic, staging, or surveillance test a job it cannot perform.

Colonoscopy with biopsy diagnoses the primary lesion; imaging and operative pathology establish extent.

CEA can support prognosis and post-treatment surveillance when elevated at baseline, but it is neither a screening test nor proof of recurrence by itself.

A rising CEA should trigger clinical and imaging evaluation rather than automatic treatment without localization.
